[摘要] 利用1HNMR为基础的代谢组学技术,探讨牛黄解毒片中对雄黄具有配伍减毒作用的主要化学成分群。将24只大鼠随机分成4组:空白组、R组(雄黄组)、RRSPG组(雄黄配伍大黄、黄芩、桔梗和甘草组)、RC组(雄黄配伍大黄总蒽醌、黄芩总黄酮、桔梗总皂苷、甘草总黄酮和甘草总皂苷组),对各组大鼠尿液及血清样品1HNMR谱图进行模式识别分析。结果显示雄黄组大鼠尿液及血清样品代谢轮廓和空白组差异明显,RC组大鼠代谢轮廓同空白组相似,向空白组回归,RC组各化学成分可通过调节能量代谢、胆碱代谢、氨基酸代谢和肠道微生物代谢等方面而起到降低雄黄毒性的作用。

[Abstract] To study the chemical component groups with toxicity alleviation effect to Realgar in Niuhuang Jiedu tablet based on1HNMR metabonomics Twentyfour male Wistar rats were divided into four groups: control group, R group (treated with Realgar), RRSPG group (treated with Realgar, the root and rhizoma of Rheum palmatum, the root of Scutellaria baicalensis, the root of Platycodon grandiflorum and the root and rhizoma of Glycyrrhiza uralensis) and RC group (treated with total anthraquinones from the root and rhizoma of R. palmatum, total flavonoids from the root of S. baicalensis, total saponins from the root of P. grandiflorum, total flavonoids and saponins from the root and rhizoma of G. uralensis) Based on1HNMR spectra of urine and serum from rats, PLSDA was performed to identify different metabolic profilesThe metabolic profiles of R group were different from that of control group, while the metabolic profiles of RC group were almost similar to control groupTotal anthraquinones from the root and rhizoma of R. palmatum, total flavonoids from the root of S. baicalensis, total saponins from the root of P. grandiflorum, total flavonoids and saponins from the root and rhizoma of G. uralensis regulated energy, choline and amino acid metabolism and gut flora disorder affected by realgar′s toxicity

牛黄解毒片是清热解毒名方,始见于元代《咽喉脉证通论》一书,被2015年版《中国药典》一部收载。牛黄解毒片由人工牛黄、雄黄、冰片、石膏、大黄、黄芩、桔梗和甘草组成,用于治疗火热内盛,咽喉肿痛,牙龈肿痛,口舌生疮,目赤肿痛等。砷是国际癌症中心确认的人类致癌物之一,可引起皮肤癌、肺癌、膀胱癌和肾癌[1]。牛黄解毒片每日服用量中雄黄的含量高达200~300 mg,相当于药典规定的雄黄最高服用量的2~3倍。作为含砷复方牛黄解毒片的安全性问题已引起国内外药品监管部门的关注[2]。

近年来,国内外开始利用代谢组学技术进行早期的药物毒性研究。主要利用HPLCMS,NMR等测定生物体体液中内源性代谢产物含量,研究生物体系代谢途径,从而反映出中毒或代谢损害而引起的细胞功能异常。前期研究结果表明,牛黄解毒片几乎不呈现雄黄样肝肾毒性;牛黄解毒片组方配伍后,大黄、黄芩、桔梗和甘草共同起到了降低雄黄肝肾毒性的作用。本实验继续采用基于1HNMR代谢组学技术研究大黄、黄芩、桔梗和甘草中的主要成分大黄总蒽醌、黄芩总黄酮、桔梗总皂苷、甘草总黄酮和甘草总皂苷与雄黄配伍后雄黄的毒性,进一步阐释牛黄解毒片中可能对雄黄具有配伍减毒作用的主要化学成分群。
